  • Do I need a Physicians Referral?
    Often you do NOT need a referral to come to Physical Therapy. Call our office for more details or to schedule an appointment.
  • What Insurances are accepted?
    Activecare Physical Therapy accepts most major insurance plans. Coverage, however, can vary greatly between different policies. We can assist you in contacting your insurance provider to understand your coverage better.
  • What should I wear/bring to my appointment?
    List of medications Drivers License Insurance card Prescription for PT if applicable Comfortable clothing Relevant medical imaging
  • What is the parking situation at Activecare on Peach Street?
    There are several handicapped spots available and many others very close to the front entrance. All spaces are on ground level so no ramps or stairs, nor elevators will be encountered! If you are coming by LIFT service, they can drop you off and pick you up at the front door! Just ask and one of our amazing team members can escort you to/from your vehicle!

New patients will find our office staff to be friendly, inviting and you will feel at home the moment you walk through the door or speak with us on the phone.

 

We encourage you to bring the following items with you on the day of your appointment:

-- Doctor's written prescription for

physical therapy, if you have been given one.

-- Insurance card(s)
-- Photo ID

-- Current list of medications

-- X-ray, MRI or other test results, CD-ROMs

and reports related to the current condition
